Practice Policies & Membership Guidelines

Insurance Policies

In order to server our patients in the most cost effective way, and be able to care for you with any and all ways necessary to get you better and back to health, we do not take health insurance as a form of payment in our office.

Can I use my HSA/FSA for my membership?  

Yes! Most of our functional medicine and chiropractic services are HSA/FSA eligible. We provide itemized receipts (Superbills) upon request to help you maximize your benefits.

We accept: HSA’s, Flex Spending, cash, and all major credit cards. We apologize for any inconvenience this may cause you. For those of you that want to do your own submitting for reimbursement, we have super bills that are directly uploaded on your patient portal that you will be able to submit to the appropriate insurances on your own. Make sure you contact them directly to ensure you can submit for reimbursement.

Check Policies

We do not accept checks until you are a client/patient for (6) visits. No exception. We do however accept all major credit cards: Visa, MasterCard, Discover and American Express.

Refund Polocies

All credit card and/or debit card refunds will be the original transaction amount less the services used and payment processing fees. All cash and/or check refunds will issued as a check for the original transaction amount less any services used.

Credit Card Fee

Due to the increase in fees in our credit card companies and the other fees tacked on to our business. We are choosing to charge a 3% credit or debit card fee when paying your invoice or service in office effective 1/28/25. This makes it so we don’t have to raise our fees for service. Thank you for understanding.

How to avoid the fees: pay with cash in office or if you have been with us for awhile you can also opt to pay with a check.

Returned Item Fee

There will be a $30 Non-Sufficient Funds fee if your credit card, debit card, or check is sent back to us as a returned item from your financial institution.

Lab Testing

No tests will be ordered for any person who is not under the direct care and supervision of Dr. Nicole Klersy-Mohr. Lab tests are one of the many tools Dr. Nicole utilizes to maintain the best possible care for her patients. They are only ordered for active patients whom are currently following treatment plans designed specifically for them by Dr. Nicole. Classroom rules & policies

All classes offered through Integrative Concierge are first come first serve, and you must register online either through our website or on the Step up for Students website. There is limited space in each class to keep the experience high for each learner. We try to maintain 12 students to one teacher ratio.

For students to be enrolled, they must pay in one of the ways listed above, and also receive a confirmation of enrollment. All fees charged and fees due are non-refundable once the class begins.

Sibling discounts at this time are not something we offer. We are keeping the fees low to ensure all families can join in. Each child will receive their own projects, take home handouts and hands on activities.

We reserve the right to cancel or postpone a class due to weather such as a hurricane, instructor illness or unforeseen circumstances, based on the circumstance. It is not required to make up the curriculum, but an option based on availability of learners and instructors.
